Claims Management

The claim information is received at our service desk when the client launches a claim.

Types of Claims

 

Partial Maturity

Process of Claiming

  • A letter is addressed to the policyholder informing him/her of the partial maturity that is due. 
  • Submit a photocopy of your policy document along with the filled claim form; provide the copy of your identification card or your passport and a copy of current pay slip showing the insurance deductions.
  • The claimant will be contacted and requested for an account number in which the sum payable will be deposited.

 

Full Maturity

On maturity we will pay to you the sum assured as agreed on the policy document.

Process of Claiming

  • A letter is addressed to the policyholder informing him/her of the full maturity that is due to him.
  • Submit your original policy document along with the filled claim form; provide the copy of your identification card or your passport and a copy of current pay slip showing the insurance deductions.
  • We will contact you requesting for your account number to deposit the sum payable.
 
Surrender

Upon written request the insured may surrender this policy for its cash surrender value. The contract will terminate on the date of the request.

Process of Claiming

  • Send a letter of notification to surrender/termination indicating the intention to disengage.
  • Submit your original policy document along with the filled claim form; provide the copy of your identification card or your passport and a copy of current pay slip showing the insurance deductions.
  • Our company will contact requesting for your account number to deposit the money into your bank.

 

Death Claim

The claimant launches a claim through a written request in case the life insured dies either through natural or accidental death.

Process of Claiming

A letter of notification of the life assured is received from the claimant
The following documents are submitted to our office.
  • The original policy document.
  • Original death certificate to be verified by the office
  • Letter of surrender of ID.
  • Letter from the chief indicating relationship of deceased and the beneficiaries
  • Copy of the beneficiaries’ ID
  •  Copy of last pay slip showing the insurance deductions
Complete the following forms
  • Claim and discharge voucher form to be completed by the claimant
  • Physician’s statement form to be completed by the deceased personal medical attendant or doctor who treated the deceased before death occurred.
  • Duly filled claim/ inquiry form.
Our company will contact requesting for your account number to deposit the money into your bank.

Note:

If the claimant has lost the policy document, he/she completes an affidavit at his own expense. The affidavit is then used in place of the original policy document.

The documents are received at the front desk, recorded in the claims register forwarded to the managing director for perusal who then forwards them to claims personnel for action or filed immediately in the file. A soft record of incoming claim mail is kept by claim personnel for reference.
